Benzoyl peroxide is one of the best-supported acne ingredients you can buy without a prescription. It kills acne-causing bacteria, helps loosen clogged pores, and, unlike antibiotics, bacteria don’t appear to become resistant to it. A 2.5% product appears to work about as well as 10% for inflamed spots with less irritation. The 2024–2025 benzene headlines are real but, based on FDA testing, don’t mean you need to stop using it; storing it properly matters more.
What is benzoyl peroxide?
Benzoyl peroxide (often shortened to BPO) is an antibacterial acne medicine found in washes, gels, creams and leave-on spot treatments. Under the FDA’s OTC acne monograph, non-prescription products can contain benzoyl peroxide at 2.5 to 10 percent. According to StatPearls, it’s available in 2.5%, 5% and 10% strengths, both over the counter and in prescription formulas, including combination products with antibiotics or retinoids.

How does benzoyl peroxide work?
Once on the skin, benzoyl peroxide breaks down and releases oxygen free radicals that damage bacterial proteins. StatPearls describes it as bactericidal against Cutibacterium acnes (the bacterium involved in inflamed acne), with mild effects on oil and on the build-up of skin cells that plugs pores.
The feature that sets it apart is resistance. Unlike with antibiotics, drug resistance has not appeared to develop with benzoyl peroxide. That’s why guidelines pair it with antibiotics (more on that below).
What does the evidence show?
The 2024 American Academy of Dermatology (AAD) guideline gives benzoyl peroxide a strong recommendation for acne. The best-quality reviews, though, are more cautious than the guideline’s confidence might suggest:
- A 2020 Cochrane review of 120 studies (29,592 people) found low-certainty evidence that benzoyl peroxide may increase self-reported treatment success compared with placebo or no treatment. It found there may be little to no difference between benzoyl peroxide and adapalene, and that people using benzoyl peroxide were more likely to stop treatment, mostly because of redness, itch and burning.
- A 2021 network meta-analysis in the British Journal of Dermatology (40 trials, 18,089 participants) estimated that 35% of people using benzoyl peroxide reported improvement versus 26% on vehicle (an inactive base). Combining it with adapalene (54%) or clindamycin (49%) ranked as more effective than benzoyl peroxide alone.
The takeaway: benzoyl peroxide helps, the evidence isn’t as high-quality as you might hope, and it tends to work best as part of a combination.
Is 2.5% as good as 10%?
For inflamed spots, the limited evidence says yes, more or less. In a set of three double-blind studies involving 153 patients, 2.5% benzoyl peroxide gel was more effective than its vehicle and equivalent to 5% and 10% gels at reducing inflammatory lesions (papules and pustules). Peeling, redness and burning were less frequent with 2.5% than with 10%. These are old, small studies, so treat the finding as reassuring rather than definitive.
That fits current advice. The AAD’s adult acne guide says that while you’ll find products with up to 10% benzoyl peroxide, “it’s best to start with a product that contains 2.5%.” In coverage of the 2024 guideline, a guideline author noted that lower strengths tend to be less irritating.
Why it’s paired with antibiotics
Antibiotic resistance is a real problem in acne care. The AAD guideline’s good-practice statements include limiting systemic antibiotic use and combining systemic antibiotics with benzoyl peroxide and other topicals. The AAD’s patient page explains that using benzoyl peroxide alongside an antibiotic “reduces your risk of developing antibiotic resistance.”
UK practice is similar. The NICE-based pathway summarized by NHS Scotland advises against topical antibiotic monotherapy, lists benzoyl peroxide with clindamycin as a first-line fixed combination, and suggests benzoyl peroxide on its own if other first-line options are unsuitable or you want to avoid retinoids and antibiotics. If you’ve been prescribed a topical antibiotic, ask whether you should be using benzoyl peroxide with it.
The benzene question: what actually happened
This is the part most people are worried about, so here is the timeline as the primary sources describe it. FDA describes benzene as “a known human carcinogen that causes leukemia and other blood disorders,” and says benzene levels in drug products should be restricted to 2 parts per million (ppm) unless otherwise justified.

The single most important statement is FDA’s own. In its March 11, 2025 announcement, the agency said: “Even with daily use of these products for decades, the risk of a person developing cancer because of exposure to benzene found in these products is very low.” The recalls were at the retail level (stores pulled products), not the consumer level, and FDA advised checking expiration dates and throwing out expired products. It did not tell people to stop using benzoyl peroxide.
The AAD’s position has stayed consistent. In March 2024 it suggested that people who were worried could switch to topical retinoids, salicylic acid or azelaic acid, and in March 2025 it said there was “no immediate cause for concern” for people who had a recalled product. Its benzene FAQ gives practical storage advice:
- Store benzoyl peroxide products at room temperature or cooler (the fridge is fine).
- Replace them every 10 to 12 weeks.
- Throw out products exposed to higher temperatures, such as above 78°F (for example, left in a hot car).
My read: the heat-and-age link is plausible and cheap to act on, so follow the storage advice. But the regulator that tested the products found most were fine, and the early studies of actual users are reassuring.
How to use benzoyl peroxide
- Start low. A 2.5% to 5% product is enough for most people, based on the strength comparison above.
- Choose wash or leave-on. A wash is rinsed off after a short contact time, which many people find easier to manage on the chest and back; leave-on gels stay on the skin longer. For body breakouts, see back acne.
- Patch-test. FDA advises applying a new OTC acne product to one or two small areas for 3 days first.
- Protect your fabrics. The monograph label warns to avoid contact with hair and dyed fabrics, which may be bleached. Use white towels and pillowcases.
- Use sunscreen. The label directions say to apply sunscreen after using this product if going outside.
- Give it time. The NHS says most people need a 6-week course of treatment.

Side effects and who should avoid it
Dryness, redness, peeling and stinging are common, especially early on and at higher strengths. The NHS lists dry and tense skin and a burning, itching or stinging sensation. StatPearls notes that a small number of people develop contact dermatitis with marked redness and irritation, which is a reason to stop.
The OTC label says not to use it if you have very sensitive skin or are sensitive to benzoyl peroxide, and to stop and ask a doctor if irritation becomes severe. Rarely, reactions are serious: FDA’s 2014 safety communication identified 131 cases of hypersensitivity reactions to OTC benzoyl peroxide or salicylic acid products between 1969 and January 2013, including throat tightness, trouble breathing and facial swelling. These need emergency care.
Pregnancy: the AAD says experts consider benzoyl peroxide safe in limited amounts during pregnancy, and suggests discussing it with your obstetrician or dermatologist first. The MotherToBaby fact sheet notes that the American College of Obstetricians and Gynecologists lists it among topical options that may be used in pregnancy.
When to see a dermatologist: if you’ve used benzoyl peroxide consistently for 6 to 8 weeks with no improvement; if you have deep, painful nodules or cysts, or any scarring; if irritation is severe or you notice swelling, hives or trouble breathing (seek urgent care for these); or if you’re pregnant or breastfeeding and want a tailored plan. A dermatologist can also discuss prescription combinations that pair benzoyl peroxide with a retinoid or antibiotic.
