| Verdict | Fine together, with one caveat |
| What’s at stake | One specific thing to get right. The detail is below. |
| How to use them | Fine to use together, and plausibly helpful. Just be aware the honest version of the claim is ‘barrier-supporting moisturisers containing niacinamide improve tolerability of topical acne treatment’, not ‘niacinamide cancels retinoid irritation’. |

Where each one stands on its own
| Ingredient | What it does | Guideline standing |
|---|---|---|
| Tretinoin | A prescription retinoid. Normalises how skin cells shed, so pores clog less in the first place. | Strong recommendation in the 2024 AAD guideline, on moderate-certainty evidence. |
| Niacinamide | A barrier-supporting vitamin B3 derivative. Popular, well tolerated, and thinly evidenced for acne. | The guideline found insufficient evidence to recommend for or against it. |
What the evidence actually says
We could not find a trial of niacinamide added specifically to a retinoid. What exists is adjacent: a split-face double-blind randomised trial found a moisturiser containing ceramides and niacinamide, used alongside topical acne treatment, beat a plain cream on lesion counts, hydration, sebum and skin pH, and concluded it decreased cutaneous irritation. Niacinamide is also known to support the skin barrier components that retinoids disrupt.
So how should you actually use them?
Fine to use together, and plausibly helpful. Just be aware the honest version of the claim is ‘barrier-supporting moisturisers containing niacinamide improve tolerability of topical acne treatment’, not ‘niacinamide cancels retinoid irritation’.
