What the evidence actually says
This is the strongest evidence on this page, and it runs the opposite way to the usual ‘don’t mix’ instinct. Benzoyl peroxide has no documented resistance in the bacteria involved in acne, so it suppresses the resistant population that antibiotic monotherapy selects for. In one clinical comparison, clindamycin-resistant bacterial counts rose to over 1600% of baseline on clindamycin alone, but stayed at or below baseline when benzoyl peroxide was added. The AAD guideline makes fixed-dose antibiotic-plus-benzoyl-peroxide its own strong recommendation, and separately notes that topical antibiotic monotherapy is not recommended.
